Emergency Form

You are required to fill one out to volunteer, not for general membership.  These Emergency Medical Forms are to be turned in to Kim Kovesci and will be kept in a secure file in the office.  These forms are only needed in case of a medical emergency.

Members Volunteer Hours Entry Form

This digital form, the same as the digital sign-in forms located at the museum, is for MAPS Air Museum volunteers to enter in their volunteer time. You will need to be already logged into your “MAPS” Google account (not your personal account) prior to entering in the information.
